29 CFR 1910.119(i)(1): The employer did not perform a pre-startup safety review for new facilities and for modified facilities when modification is significant enough to require a change in the process safety information: a) Enterprise Products Operating, LP at 1250 County Road 69, Rifle, CO 81650: On and before 8/1/13, the employer did not perform a pre-startup safety review for modified facilities when modification is significant enough to require a change in the process safety information. The employer replaced the Amine sump with a 1000 gallon vessel, replace flash gas line and reroute valve without a PPSR. This condition exposed employees to chemical, fire and explosion hazards.
