29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Dawson Grain Handling Facility, on or about and at times proceeding to July 23, 2013, fall protection was not provided for employees accessing the port hole at the tops of the grain bins to verify the storage inventory levels, exposing employees up to an 80 foot fall hazard.
