29 CFR 1960.8(d): The Agency did not acquire safety equipment necessary to protect employees: Anacapa Island Landing: On or about 10JUL13, the employer did not provide equipment, such as a gangway and platform, allowing employees to safely board and disembark from vessels to the landing; an employee was fatally injured falling from the ladder while boarding the vessel from the landing, striking his head and suffering additional injuries when he was caught between the vessel and the pier pilings when he landed in the water below.
