29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: Bottle Line # 1 Palletizer: A barrier guard which was loosely hanging in place was not affixed and secured to the metal wall of the machine to prevent removal by employees. An employee removed the hanging barrier without using a tool and reached into the machine to straighten out a mis-fed pallet. The employee was struck when his action prompted the machine to resume operation before he could remove himself from the path of a hoisting mechanism.
