29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used: a) Building #1500 - On or about June 27, 2013, employees working on the second and third levels of a three tier tubular welded frame scaffold accessed the upper levels that were more than 2 feet above or below a point of access by climbing the ends of the scaffold, which were not equipped with suitable means of access, exposing employees to fall hazards of up to approximately 20 feet onto earthen ground. ABATEMENT CERTIFICATION IS NOT REQUIRED
