29 CFR 1910.303(b)(2): The 4-plex electrical box was not installed and used in accordance with any instructions included in the listing or labeling: (a) Gold Plane Custom Cabinets, LLC, 11170 Irma Drive, Northglenn, CO 80233: On and before June 13, 2013, the employer used a 4-plex electrical box equipped with two duplex outlets as the source of electrical power for the Gannomat Model 280 dowel machine. The flexible power cord for the Gannomat machine was wired directly into the 4-plex box. The electrical box is not designed to be used in this manner. This condition exposed employees to potential electrical hazards.
