29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system: a) Utility Work Site: On or about April 26, 2013 and at times prior, employees were exposed to a trench cave in while replacing a concrete fiber optic vault in an excavation approximately six foot, seven inches deep with no cave-in protection.
