29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). (a) Trench, new storm water management facility located between Lots #5 & #6 on South Sherwood Road, Smyrna, GA 30082: Employee worked in a trench which measured 6-feet wide by 6-feet, 9-inches deep by 8-feet long grouting a 24" concrete sewer line to an existing sewer main without a cave-in protective system in place. (b) Trench, new storm water management facility located between Lots #5 & #6 on South Sherwood Road, Smyrna, GA 30082: Employee worked in a trench which measured 5-feet, 10-inches wide by 6-feet deep by 40-feet long installing concrete sewer lines without a cave-in protective system in place.
