29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 24, 2013, employees performing decking installation were using a ladder that did not extend three feet above the landing to access and egress the roof, exposing the employees to a fall hazard of up to nine feet to the ground below. Alvaro Lozano was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 312124175, citation number 1, item number 4, and was affirmed as a final order on August 12, 2009 with respect to a workplace located at 1518 Havenrock Drive, Forney, TX 75126.
