29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to meter vault #2 - building #7, on or about 4/11/13. Employees entered the confined space without the completion of an entry permit. b. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe using the ladder near sedimentation tanks # 5-8, on or about 4/10/13. An employee carried a 4" vacuum hose approximately 5- 20 ft inside the pipe to pump out water.
