29 CFR 1910.269(d)(6)(vii): Before starting work on machines or equipment that have been locked out or tagged out, the authorized employee did not verify that isolation and deenergizing of the machine or equipment had been accomplished: At the AMEREN-UE Calloway Nuclear Plant, switchyard, specifically at Bus B, 52-4, an arc flash occurred on or about 04/02/13 severely burning employees. Verification that Bus B, 52-4 was isolated from its 13.8kv electrical source was not performed prior to employees attempting to place personnel protective grounds on connectors located inside of this piece of equipment.
