29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold more than 10 feet above a lower level was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) 1718 North 42nd Street, Philadelphia, PA- On December 19, 2012 employees installed aluminum flashing/capping from the top level of a tubular steel frame scaffold. The planked working level was over 32 feet above the sidewalk below and no standard guardrails were installed or other equivalent fall protection was provided. Conditions noted on December 19, 2012.
