29 CFR 1926.1053(a)(16): The normal step across distance for through fixed ladders at their point of access/egress exceeded 12 inches (30 cm) and a landing platform was not provided with a step across distance of not less than 7 inches (18 cm) nor more than 12 inches (30 cm) as measured from the centerline of the steps or rungs to the nearest edge of the landing: a) Water Tower, Exterior Entry Ladder -- On or about December 5, 2012, employees were using an extension ladder to access the a water tower's exterior mounted ladder cage that did not have a landing platform. This required the workers to step across more than 12 inches to reach the access point of a fixed ladder located no less than eight feet above grade, without any means of fall protection.
