29 CFR 1910.219(i)(2): Revolving surfaces of shaft coupling(s) were not covered by a safety sleeve: a. On or about October 1, 2012, in the asphalt manufacturing area, the fuel oil service pumps on the asphalt heater assembly did not have a coupling guard, exposing employees to being caught by rotating machinery. b. On or about October 1, 2012, in the asphalt manufacturing area, the fuel oil transfer pumps on the fuel oil service tanks did not have a coupling guard, exposing employees to being caught by rotating machinery.
