29 CFR 1910.119(e)(5): The employer did not assure that PHA recommendations were resolved in a timely manner: a) Old Acid Room Area - Employees are exposed to a potentially catastrophic release of Formaldehyde from the employer not addressing PHA recommendations in a timely manner, on or about September 26, 2012. The employer did not address the following recommendations from the 1997 initial PHA: 1) Implementing a preventative maintenance (PM) schedule to address 316 S/S piping leakage. 2) Implementing a preventative maintenance (PM) schedule to address 316 S/S flange leakage. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
