29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: a) On or about April 26, 2012, the employer did not ensure that the electrical house service that presented a electrical shock potential was first de-energized and isolated. The employee entered into an area of the crawl space where the electrical equipment was energized and afetr a water line was broke, increased the electric shock potential in and around the employees work zone.
