29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions: A)Chlorine Repackaging Line - On October 14, 2011, and at times prior thereto, the Scott/Bacharach Series 4600 Chlorine Sensor was not installed according to the manufacturer's instructions, in that the sensor was installed approximately four feet above the floor instead of 18 inches above the floor. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
