SAVE: 29 CFR 1910.119(e)(3)(ii): The process hazard analysis did not address any previous incidents which had a likely potential for catastrophic consequences i n the workplace. The employer does not ensure that previous incidents are consid ered when performing PHAs. This violation was observed on or about March 3, 2010 . The employer did not include incident investigations when conducting the 2009 PHA revalidation for th e Platformer. There were 6 incidents in the Platformer in 2007; 1 in 2008 (a fla nge fire on Heater 1 on 01/15/2010); and 1 in 2009 (a fire in the outlet of Heat er 3 on 3/21/2010). None of these incidents were considered during the 2009 PHA Revalidation. A failure to consider past incidents that could be repeated potent ially exposes employees to fire and explosion hazards. Pursuant to 29 C. F. R. 1 903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taking to ensure previous incidents are included in all PHAs. One method of abatement among others is to develop a mechanism that pr ompts the PHA team who is preparing for a PHA to search for past incidents.
