29 CFR 1910.106(c)(4): Piping systems were not substantially supported and protected against physical damage and excessive stresses arising from settlement, vibration, expansion, or contraction: a- Hess Port Reading Refinery, Gas Con - The steel pipe support for the Disulfide Oil (DSO) line off E-605B Sponge Oil Cooler was not bolted in place leaving a gap of 0.75 in. between the flange and concrete footing. Condition noted on or about 12/18/08. b- Hess Port Reading Refinery, Gas Con - The flare line for the E-640 Lean Gas Cooler was inadequately supported with noticeable vibration. Condition noted on or about 12/18/08. c- Hess Port Reading Refinery, Alkylation (Alky) - The 6 inch overhead line from the E 711C Feed Effluent Exchanger was supported by a piece of wood and wire wrapped around an above pipe. The same line was being struck by an adjacent light fixture and conduit moving in the wind. Condition noted on or about 02/27/09. d- Hess Port Reading Refinery, Alky R-701 Contactor - Steel pipe supports associated with PC-047/PV-047 were not adequately secured to the work floor with one support anchored with only a single bolt and another not bolted to floor. Condition noted on or about 02/27/09. e- Hess Port Reading Refinery, Alky - Steel pipe supports for E-711A Feed/Effluent Exchanger were not secured with anchor bolts with a gap of up to 3 inches between the flange and work floor. Condition noted on or about 02/27/09. f- Hess Port Reading Refinery, Alky R-702 Contactor - The steel pipe support near PC-055/PV-055 was supported by a broken footing with no anchor bolts. Condition noted on or about 02/27/09. g- Hess Port Reading Refinery, Alky T-703 Debutanizer - The two inch insulated feed piping running to the top of the tower was inadequately supported with noticeable vibration. Condition noted on or about 02/27/09.
