Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a)Integrity Trucking LLC at Merit Well Girltey 31-23 WCR 38 - 25.5, Gilcrest, CO 80623: On or before October 24, 2008, the employer did not ensure that the operator of the base-mounted winch on the 2000 Freightliner Classic truck, serial number 1FUPCXZB6YPF04439 was qualified to operate the winch by passing a written or oral examination and a practical operating examination. Condition exposed employees to struck by hazards. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
