Text For Citation: 01 Item/Group: 004 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Acts of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to the conditions listed below: a.Drilling Site. The employer did not successfully train employees in proper rigging techniques, in areas such as, but not limited to, the installation and pull testing of ground anchors/stakes, proper guylines patterns procedures, and inspecting surface ground conditions that might indicate reduced anchor capacity in that employees were exposed to the hazard of collapse of the derrick/mast on the Rambler Rig #21, serial number 689. Employees were performing work off of the rig floor and pipe racking board, as determined on or about 05/17/08. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT TO CORRECT THIS HAZARD IS TO ENSURE THAT EACH EMPLOYEE BE TRAINED IN PROPER RIGGING TECHNIQUES THAT INCLUDES THE RECOGNITION AND AVOIDANCE OF UNSAFE CONDITIONS, AND IN THE RULES AND REGULATIONS APPLICABLE TO HIS WORK ENVIRONMENT TO CONTROL OR ELIMINATE ANY OCCUPATIONAL HAZARDS OR OTHER EXPOSURE TO ILLNESS AND INJURY, AS SPECIFIED IN AMERICAN PETROLEUM INSTITUTE (API) 1988, SPECIFICATION 4E, SPECIFICATION FOR DRILLING AND WELL SERVICING STRUCTURES. SUITABLE TRAINING SHALL INCLUDE REQUIRING OPERATORS AND SUPERVISORS THAT IT IS THEIR RESPONSIBILITY TO ENSURE THAT UNDER NO CIRCUMSTANCE WILL THE UNIT WILL BE RIG UP TO ANY ANCHOR THAT IS NOT PULL TESTED OR THE RIG GUYING PATTERN WAS NOT PROPERLY FOLLOWED.
