On 11/21/07, the employees did not effectively communicate to all other employees performing work in the area that an employee was operating and monitoring a HT2000 3512B Pump connected to a manifold truck (Unit #10864618) to set a pump seal when the line became pressurized. Among others, feasible means of abatement could include: Ensure that all employees and supervisors involved in pressuring a line are in effective communication with each other through communication equipment or hand signals and are familiar with communication and safety procedures.on
