Text For Citation: 01 Item/Group: 004 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing injuries: a) Northwest Pipe Company, 6030 N. Washington Street, Denver, CO: Located in the Spiral Mill building, control panels "A&B" and B&C were wired/configured in such a way to allow operators to "reset" or "restart" conveyors "A","B" and "C" that had been stopped due to activation of emergency stop light sensors without knowing if an employee was in the path of conveyor travel. b) The employer had not ensured that operators of conveyor control stations ("A&B" "B&C") physically inspected conveyors prior to restarting those where movement had stopped possibly due to activation of an emergency light sensor(s). Employees are instructed to push the "reset" button on the conveyor control panel in the event that conveyors will not start for reasons unknown. Among other methods, on feasible and acceptable abatement method to correct the hazard listed in paragraph a) above is to adopt the American Society of Mechanical Engineers ASME B20.1-2000 titled Safety Standard for Conveyors and Related Equipment. Specifically, paragraph 5.11.4 states the following: "Conveyor controls shall be so arranged that, in case of emergency stop, manual reset or start at the location where the emergency stop was initiated shall be required for the conveyor(s) and associated equipment to resume operation". Among other methods, one feasible and acceptable abatement method to correct the hazard listed in paragraph b) above is to adopt the American Society of Mechanical Engineers ASME B20.1-2000 titled Safety Standard for Conveyors and Related Equipment. Specifically, paragraph 5.11.4 states the following: "Before restarting a conveyor that has been stopped because of an emergency, an inspection of the conveyor shall be made and the cause of the stoppage determined".
