Text For Citation: 01 Item/Group: 004 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)the hazard of being struck by well tooling, pump rods and/or portions of the rig structure of the #1 Workover Rig, located at the Edelblute Number 1 well, Powerline Road, Rosenberg, TX, following failure of the hoist line due to a "tow block" situation during operations. Among other methods, one feasible and acceptable abatement method to correct the hazard is to provide an upper hoist limit switch to limit the upward travel motion of the hoist block and well string, as specified by Section 9.4.8 of the American Petroleum Institute (API) Standard Recommended Practice, RP 54-1999, "Recommended Practice for Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations." DISCLAIMERS:1) You are not limited to the abatement methods suggested above. 2) The methods explained are general and may not be effective in all cases. 3) The employer is responsible for selecting and carrying out an appropriate abatement method.
