29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process: a) Establishment - P&ID 112A and P&ID 107B for Condensers D7 & D8 did not show 6 -HSD-207 going through the wall. On or about 2/8/12. b) Establishment - P&ID 104D for Freezer F5 shows 1 ? -HPSCL-700-3 line from Freezer F4 leading into Freezer F5, but there is no line on P&ID 104B for Freezer F4. It should show 1 ? HPSCL-700-3 from Intermediate Accumulators A1 and A2 P&ID 103A. On or about 2/8/12. c) Establishment - P&ID 103F for Pump Recirculator A11 does not show ICS line coming off of the vessel. On or about 2/8/12. d) Establishment - P&ID 103F for Pump Recirculator A11 shows 2 -HPSCL-762-3 from P&ID 104F for Freezer F4 and 3 -LTRL-708-3 to P&ID104F for Freezer F4. The lines both should show Freezer F8 not F4 as P&ID 104F is Freezer F8. On or about 2/8/12. e) Establishment - P&ID 103F for Pump Recirculator A11 shows 2 -HPSCL-762-3 from P&ID 104F for Freezer F4 which should show the line as ICLS. On or about 2/8/12. f) Establishment - P&ID 105A shows a ? -HG-811 line that goes to Freezer F5 on P&ID 104D, there is no HG line that goes to Freezer F5 on P&ID 104D. On or about 2/8/12. g) Establishment - P&ID 105A shows a 1 ? -HG-803 that comes from Condensers D1-D6, Condensers D5 and D6 were decommissioned, P&ID should show Condensers D1-D4. On or about 2/8/12. h) Establishment - P&ID 110B- 1 ? -HPSCL-709 from Flash Intercooler A8 should show ICLS instead of HPSCL. On or about 2/8/12. i) Establishment - P&ID 112A line that shows to 3 -HPL-307 to Receivers R1 and R2 on P&ID 102A should show to 3 -HPL-358 to EQ on P&ID 102A. On or about 2/8/12 j) Establishment - P&ID 103A line 2 -HPL-314-2 to A3 P&ID 103B should show ICLS not HPL on P&IDs 103A and P&ID 103B. On or about 2/8/12. k) Establishment - P&ID 103C outgoing line from Accumulator A6 2 -HPSCL-701 to A7 on P&ID 103D should show ICLS not HPSCL on P&ID 103C and P&ID103D. On or about 2/8/12. l) Establishment - P&ID 103E line from Freezer F5 1 ? -HPSCL-700-3 P&ID 104D should show ICLS not HPSCL. On or about 2/8/12. m) Establishment - P&ID 105A line 6 -LSS-508-3 -SP from Plate Freezers 1 and 3 to Freezer F8 should show Freezer F7. On or about 2/8/12. n) Establishment - The ball valves on each of the oil pots and oil legs were not shown on any of the P&IDs. On or about 2/8/12.
