29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i) through (f)(1)(v) a) On or about October 15, 2025, the employer did not ensure employees were protected from struck-by hazards while installing a Century 5030T tow truck extension boom hydraulic cylinder. On or about October 15, 2025, an employee was caught between the hydraulic cylinder base and frame of the tow truck while positioning it to install the pin.
