29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a.) a.) At the worksite, located at 2307 E Busch Blvd Tampa, FL 33612; On or about 8/19/25, the employer exposed employees to fall hazards of approximately 16ft in that an extension ladder did not extend 3 ft past the landing surface for employees conducting roofing operations on a commercial building.
