29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment: a) In the 13, 14 Battery Transfer Area Pad Valve Pit, on or about August 11, 2025 - The employer did not establish written procedures to maintain the on-going integrity of 13 Pad Valve. This exposed employees washing the valve, as well as other employees working on or adjacent to the 13, 14 Battery Transfer Area to an explosion when 13 Pad Valve ruptured during washing and released cryogenically-processed coke oven gas from the Underfire lines.
