29 CFR 1910.146(d)(4)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at no cost to the employees communications equipment necessary for compliance with 29 CFR 1910.146(h)(3) and (i)(5): On or about August 1, 2025, and at times prior, on a site located at 4529 Hypoluxo Road, Lake Worth, Florida, the employer did not provide equipment necessary to communicate with employees exposed to hazardous chemical vapors such as, but not limited to Benzene and Toluene, when entering an underground fuel tank to perform a repair.
