29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Boiler Room: On or about June 30, 2025, employees were exposed to trip and fall hazards from an unprotected hole in a concrete slab measuring approximately 24 inches by 18.5 inches, in that the employer failed to cover the hole before work began near the walking/working surface.
