29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: On or about June 12, 2025, and at times thereafter, the employer did not prepare an entry permit prior to employees entering manholes to an underground vault(s) in the following instances listed below: a) On or about June 12, 2025, where one (1) employee was allowed to enter and work in an underground vault; and b) On or about June 13, 2025, where two (2) employees were allowed to enter and work in an underground vault (Vault (53+60)).
