29 CFR 1926.452(g)(1): Each bracket, except those for wooden bracket-form scaffolds, was not attached to the supporting formwork or structure by means of one or more of the following: nails; a metal stud attachment device; welding; hooking over a secured structural supporting member, with the form wales either bolted to the form or secured by snap ties or tie bolts extending through the form and securely anchored; or, for carpenters' bracket scaffolds only, by a bolt extending through to the opposite side of the structure's wall: a) On or about April 29, 2025, on the northeast side of Tank A, employees jumped/lifted a Doka Climbing Formwork MF 240 scaffold from a lower ring level to ring level 7 without ensuring that the platform was secured to the Tank A form wall.
