At the worksite located at 7475 W US HWY 60, Republic, MO 65738, employees were exposed to struck by and crushed by hazards while conducting material handling activities. The employer did not ensure that employees were using slings that were not shortened with knots, bolts, or other makeshift devices when rigging materials/products to overhead cranes.
