Protection was not provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. At 3641 Tower Ridge, Cape Girardeau, MO 63701, on March 6, 2025 and at 5415 Flatrock Trce On February 4, 2025, Employees were exposed to struck by hazards from dirt, rocks and debris within 2 feet of the edge of an excavation with no other protective means present.
