29 CFR 1915.159(c)(5): Personal fall arrest systems were not inspected prior to each use for mildew, wear, damage, and other deterioration. On or about February 23, 2025, on the starboard side of the vessel, an employee used a harness and lifeline that had cuts, abrasions, oil and dirt contamination without inspecting the fall protection system.
