29 CFR 1910.119(e)(7): The employer did not retain the process hazard analysis (PHA), updates, or renewals for each process covered and documented resolution of the recommendations described in 29 CFR 1910.119(e)(5) for the life of the process: Cargill, Inc. DBA Cargill Meat Solutions Corporation, 1505 East Burlington Avenue Fort Morgan, CO 80701: On or about March 31, 2025, the employer failed to retain process hazards analyses (PHA) for the entire life of the covered ammonia refrigeration process which exposed employees to inhalation hazards associated with ammonia for the following: (a) Initial PHA completed in 1998; (b) Updated/Revalidated PHA completed in 2002; (c) Updated/Revalidated PHA completed in 2007; (d) Updated/Revalidated PHA completed in 2012.
