29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. The employer did not annually certify that operating procedures were current and accurate. a) On or about 01/28/2025, the employer did not ensure that the high temperature recirculator (HTR2) procedure (Revision 2, date 07/19/2023) was reviewed as often as necessary to assure that they reflect current operating practice. The following items listed in the procedure are inaccurate but not limited to: 1. On page 3 of the procedure, Pressure Relief /Safety Valve (PRV/PSV) setpoint of oil pot is displayed at 400 psig. This is inaccurate and not reflected in the field. 2. On page 3 of the procedure, high level (LT) or level transmitter is listed for system shutdown and low level float switch is listed for pump shutdown. This is inconsistence with the information on page 2 of the procedure. b) On or about 01/28/2025, at Cuisine Solutions facility located in San Antonio Texas, the employer did not ensure that compressor C1 procedure (Revision 0, date 9/27/2019) was reviewed as often as necessary to assure that they reflect current operating practice. The following items are listed in the procedure are inaccurate but not limited to: 1. On page 2 of the procedure: i. High discharge pressure cutout setpoint is listed at 40 psig. This is inaccurate and not reflected in the field. ii. High discharge temperature alarm setpoint is listed at 115 F. This is inaccurate and not reflected in the field. iii. High discharge temperature cutout setpoint is listed at 210 F. This is inaccurate and not reflected in the field. 2. On page 3, the inlet of pressure safety valve (PSV) is listed as 1-in with model Shank 815. This is inaccurate and not reflected in the field. 3. On page 12, it stated "NOTE; Ensure appropriate condensers, High Pressure Receiver PRS3/HPR1 vessel, Low Temperature Recirculator PRS2/LTR1, Medium Temperature Recirculator/Intercooler PRS2/MTA/INT1 and High Temperature Accumulator PRS3/HTA vessels are online and operating properly in accordance with their respective SOPs, and ensure the overall system is aligned in accordance with the Overall System SOP, before starting compressor. Rev. 2019". This is inaccurate. c). On or about 01/28/2025, at Cuisine Solutions facility located in San Antonio Texas, the employer did not ensure that compressor C9 procedure (Revision 0, date 11/03/2022) was reviewed as often as necessary to assure that they reflect current operating practice. The following items that are listed in the procedure are inaccurate but not limited to: On page 3 of the procedure: 1. High discharge pressure cutout setpoint is listed at 200 psig.
