29 CFR 1910.1001(j)(3)(i): Building and facility owner did not determine the presence, location, and quantity of ACM and/or PACM at the worksite and exercise due diligence in complying with these requirements to inform employers and employees about the presence and location of ACM and PACM: On or about January 21, 2025, employees were exposed to friable asbestos-containing: a. thermal system insulation on pipes and fittings while working in the boiler room and the employer failed to inform employees about the presence, location, and quantity of asbestos containing material (ACM) and/or presumed asbestos containing material (PACM). b. ceiling tile while working in patient rooms and the employer failed to inform employees about the presence, location, and quantity of asbestos containing material (ACM) and/or presumed asbestos containing material (PACM).
