29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about 11/14/2024, and at times prior, at 9875 NW 79th Avenue, Hialeah Gardens, Fl 33016 a. An industrial fan (i.e. Schaefer - model 36CFO-Y s/n - 0715220003) lacked a ground prong on its attachment plug, thus exposing employees to an electrical hazard. b. An industrial fan (Uline Pedestal Fan - model H-989; sn - unknown) lacked a ground prong on its attachment plug, thus exposing employees to an electrical hazard.
