29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: At a facility located at 110 Washington Street in Winneconne, WI: a) On or about October 28, 2024, Machine Operator employees were exposed to struck-by and entanglement hazards posed by the wheel of the Wheel #2 line during production. An interlocked gate was bypassed and did not prevent the employee from entering the area behind and below the rotating wheel. b) On or about November 21, 2024, Wheel Packer employees were exposed to amputation hazards posted by the Seybold Paper Cutters (guillotines) of Wheel #1 and Wheel #2 lines during production. Employees reached under the blades of the paper cutters to advance, position, and remove the paper product to be cut by the blades. The machines were equipped with two-hand controls; however, both paper cutters had experienced downward drifting, and one experienced an unintentional cycling.
