Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) A truck mounted power winch west parking lot, a thorough inspection of all ropes were not made at least once a month and a full written, dated and signed report of rope condition kept on file and a determination made as to whether future use of the rope would constitute a safety hazard. The 3/8 inch wire rope 1500 ft. long used to lift material was not installed in the wedge socket connected to the hook so that the pulling part was directly in line with the clevis pin. This will produce a sharp bend in the rope as it enters the socket. A feasible and useful method to correct this hazard, among others, is to replace the dogs, notch section and install heavier rods to prevent their bending.
