Bldg. 6005: On or about October 17th, 2024, and at times prior thereto, the employer did not ensure that employee(s) inspected Personal fall arrest system(s) prior to each use for wear, damage, and other deterioration, and/or defective components to include fall protection harness/equipment inspections record tags were left blank. This condition potentially exposed employee(s) to an approximately 22 foot fall hazard(s).
