29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry. Site: 31-20 Northern Blvd, Chamber B, Long Island City, NY 11101. a.) On or about October 07, 2024, National Water Main Cleaning Company did not develop and implement a plan or procedure for confined space rescue where employees were cleaning sewage from a siphon that measured 120 feet in depth and 72 inches wide. The permit-required confined space contained a hazardous atmosphere such as oxygen deficiency, hydrogen sulfide, and other life-threatening gases. Abatement certification is required pursuant to 29 CFR 1903.19.
