29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves: Employees engaged in machinery activities in the fabrication department shop were exposed to laceration, amputation, caught-in and caught-between hazards in that the rotating shaft ends on both sides of the Peck, Stow & Wilcox roller machine, Model # 390-E, Serial # 3/61 were not covered to protect operators and other employees from rotating parts. a) A 3/4 inches diameter rotating shaft end located 3 feet above the floor projected 1-3/4 inches in length and was not guarded. b) A 1-inch diameter rotating shaft end located 3 feet above the floor projected 3 inches long and was not guarded.
