29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): a) Gold Medal Environmental of New Jersey Inc. - Yard area/Landfill: The drivers did not affix a lock to the battery disconnect switch, in conjunction with maintenance personnel when entering the Mc Neilus Rear Loader and Front Loader trash compactors (packers) to remove excess trash. Occurred on or about 09/04/2024.
