29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about July 16, 2024, employees who conducted servicing and maintenance activities including, but not limited to, changing the the inserts and jaws on the CNC Lathe number 47659 were exposed to electrical, hydraulic, and mechanical energy sources. Although the interlocked door was opened up to perform the work, the remaining applicable energy control elements involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from caught in hazards. b. On or about July 16, 2024, employees who conducted servicing and maintenance activities including, but not limited to, changing the blade on the Amada band saws in the Saw Department were exposed to electrical, hydraulic, and mechanical energy sources. Although the e-stop was hit and an interlocked arm was opened up to perform the work, the remaining applicable energy control elements involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from caught in hazards.
