29 CFR 1910.119(f)(3): The operating procedures were not reviewed to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities: On or about July 3, 2024, at General Dynamics Ordnance and Tactical Systems (GD-OTS) facility in Hampton Arkansas, the employer did not ensure that the "Standard Operating Procedure for M274 Smoke Tube Assembly (Perchlorate Free)" was reviewed to assure that it reflects current operating practice. The following items which were listed in the procedure contained deficiencies, including but not limited to: a.) In section 2.0 "Operation" on page 11, the employer listed an explosive load limit for Bay 4 of Building 11 was 150 pounds. However, the employer listed 100 pounds limits for Bay 4 in the facility safety data sheet of Building 11. b.) In section 2.0 "Operation" on page 11, under Bay 1 of Building 11, the employer listed "Operators" limits for 5 and "Casuals" limits for 2. However, in the standard operating procedure for "Hydra 70 Cargo Warhead Subassemblies" on page 8 "Building Load Limits" for Bay 1 of Building 11, the employer listed "Operators" limits for 10 and "Casuals" for 6. c.) In section 2.2 "Automated Smoke Tube Loading" on page 21, the employer did not list isopropyl alcohol in this section. However, employees used isopropyl alcohol for cleaning purposes.
