29 CFR 1926.251(a)(1): Defective rigging equipment was not removed from service: On or about 6/19/2024, at the jobsite located at 5145 Sesame Street, Palm Beach Gardens, FL 33418, employees were exposed to a struck by hazard while using a defective sling (i.e. Certex Nylon Sling, CEE2x94x30 ft, Serial #336132-1) to lift and pull a new pipe into an existing pipe.
