29 CFR 1926.1211(c)(2): The employer did not ensure a mechanical device was available to retrieve personnel from vertical type permit spaces more than 5 feet (1.52 meters) deep. On or about June 13, 2024, at the manhole on 6th street and Washington in Anadarko, OK, employees entered a confined space without a mechanical retrieval device.
