29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a)Comanche Stainless LLC dba Diamond Brite Metals /333 Cedar Ave/Middlesex, NJ 08846 - Shop - Employees were exposed to struck by hazards while a 800lb Metal Beam was being transported by a Yale Sit-Down propane powered forklift (Model#ERC050; Serial#A968NO5211J) utilizing an unapproved boom attachment, the Boom Attachment has no Model/Serial# as it was made by employer. The forklift truck's data and capacity plates were not changed to reflect the attachment. Condition noted on or about 6/11/2024.
